For decades, we were told the enemy was fat. Cut the cholesterol. Switch to vegetable oils. Choose the low-fat option. The food industry responded, and our shelves filled with manufactured products stripped of fat and loaded with something else entirely — sugar, processed grains, and ingredients our bodies had never encountered before.
The result was not better health. It was an epidemic of chronic disease rooted in the one thing we were never warned about: inflammation.
Gluten: The Silent Poison
Gluten is a protein found in wheat, barley, and rye. Most people associate gluten sensitivity with digestive discomfort — bloating, cramping, diarrhoea. But the gut symptoms, when they appear at all, are only the surface of the problem.
Gluten is made up of two main protein groups: glutenins and gliadins. Sensitivity can develop to either group, or to any of the twelve smaller units that make up gliadin alone. A reaction to any of these triggers inflammation — and that inflammation does not stay localised in the gut.
Gliadin in particular has been shown to damage the gut lining directly. Harvard’s Dr. Alessio Fasano, one of the world’s leading experts on intestinal permeability, put it plainly: gliadin exposure induces increased intestinal permeability in all individuals, regardless of whether or not they have coeliac disease. This is not a condition limited to those with a formal diagnosis. It is a biological response that occurs across the population.
The mechanism works like this: gliadin triggers the production of a protein called zonulin. Zonulin breaks down the tight junctions of the gut lining, increasing its permeability. Once the lining is compromised — a condition commonly called leaky gut — substances that should remain contained in the digestive tract leak into the bloodstream. The immune system, encountering these foreign particles, mounts an inflammatory response. In 2015, Dr. Fasano published a landmark paper showing how this cascade can be the underlying driver of autoimmune disorders and even certain cancers.
The brain is among the organs most vulnerable to this downstream inflammation. Gluten sensitivity — with or without coeliac disease — drives the production of inflammatory cytokines that are directly implicated in neurodegenerative conditions.
What It Feels Like — and What It Does Not Feel Like
Some people who are gluten sensitive experience clear symptoms: abdominal pain, nausea, constipation, headaches, brain fog, unusual fatigue after a gluten-containing meal, dizziness, or a persistent feeling of being slightly off balance.
Most people, however, experience nothing obvious at all — while the silent attack continues in the nervous system. Gluten can inflict lasting damage without any perceptible signal. And if left unaddressed, its effects can progress from unexplained headaches and chronic fatigue to anxiety, depression, and eventually dementia. Critically, you do not need to have digestive symptoms to have a leaky gut. The condition can manifest as skin problems like eczema or psoriasis, autoimmune disorders, heart disease, or neurological decline — with no stomach complaints whatsoever.
Inflammation, Weight, and the Cycle That Feeds Itself
For those struggling with weight, the inflammatory effects of gluten create an additional obstacle. Overweight and obesity are themselves rooted in inflammation, and the relationship is bidirectional: inflammation promotes weight gain, and weight gain promotes inflammation.
Here is how the cycle works. Elevated inflammatory cytokines in the bloodstream cause insulin resistance. This is why people with other inflammatory conditions carry a higher risk of developing type 2 diabetes — the inflammation itself is disrupting glucose metabolism. Meanwhile, fat tissue in excess of what the body requires triggers its own intracellular inflammation, which further promotes insulin resistance and additional weight gain. The cycle sustains itself.
The brain and gut are both caught in this loop. When inflammation reaches the hypothalamus, leptin resistance develops — and leptin is the hormone that controls appetite and metabolism. When leptin signalling breaks down, the brain loses its ability to accurately regulate hunger and fat metabolism.
A similar breakdown can occur in the gut. Gut inflammation causes leptin and insulin resistance, largely through the leakage of toxins into the bloodstream. One in particular — lipopolysaccharide (LPS), produced by certain gut bacteria — causes serious harm once it breaches the gut lining. It triggers widespread inflammation and insulin resistance in the liver, promoting further weight gain.
Cholesterol: The Most Misunderstood Molecule in Medicine
For decades, cholesterol was cast as the primary villain in heart disease and chronic illness. The science behind that conclusion was always more contested than public health messaging suggested — and the downstream consequences of that messaging have been severe.
Cholesterol is not an enemy. It is an essential brain nutrient. It is critical for building cellular membranes, functions as an antioxidant, and serves as a precursor to vitamin D and to steroid hormones including testosterone and oestrogen. The brain demands cholesterol in large quantities as a primary fuel source.
All of the latest science points in the same direction: when cholesterol levels are low, the brain does not function well. People with low cholesterol are at significantly greater risk for neurological problems ranging from depression to dementia.
When cholesterol became the designated villain, corporate food manufacturers responded with speed and commercial ingenuity. They produced hydrogenated fats, processed vegetable oils, and manufactured food products built on these ingredients — and labelled them “low in cholesterol” or “cholesterol-free.” What they did not advertise was that these products were dense with trans fats, substances genuinely damaging to cardiovascular and neurological health.
The shift from real food to manufactured food — undertaken in the name of health — was followed by rising rates of the very diseases it was supposed to prevent: diabetes, heart disease, and chronic inflammatory conditions across the board.
Sugar: The True Culprit in Sixty Names
While cholesterol absorbed decades of public blame, sugar quietly infiltrated nearly every packaged food on the market. It appears under more than sixty different names — cane sugar, barley malt, crystalline fructose, evaporated cane juice, caramel, high-fructose corn syrup, maltodextrin — but it is all the same substance with the same effects.
The average American now consumes 22 teaspoons of sugar per day and over 130 pounds per year. Over the past century, fructose consumption alone has increased fivefold — found naturally in fruit but consumed overwhelmingly through highly processed foods containing high-fructose corn syrup.
Fructose carries a specific set of risks. It is directly implicated in non-alcoholic fatty liver disease, a condition in which fat accumulates in the liver and triggers inflammation that can progress to scarring and cirrhosis. It is associated with insulin resistance, elevated blood fats, and high blood pressure. Most significantly, fructose is seven times more likely than glucose to form glycation end products — sticky, caramel-like aggregates of proteins and carbohydrates that cause oxidative stress and fuel inflammation throughout the body, including the brain.
Glycation, the bonding of sugar to protein, is not a minor inconvenience. It is one of the primary mechanisms through which chronic disease takes root.
The Common Thread
Gluten, excess sugar, and chronically low cholesterol share a single underlying consequence: inflammation. It begins in the gut, travels through the bloodstream, reaches the brain, and over years and decades, quietly dismantles the systems that sustain cognitive and physical health.
None of this damage announces itself clearly. There is rarely a dramatic moment of onset. There is only the slow accumulation of a fire that no one told you was burning.
Understanding what fuels it is the first step toward putting it out.
